Perimenopause and menopause are associated with accelerated aging, as changing ovarian hormones influence systems throughout the body. It may be the first time you see significant changes in your skin, joint comfort, body composition, memory difficulty, and more. And the eyes are no exception.

You may expect some vision changes as a normal part of aging, but for women, menopausal hormone shifts also play a role. If you’ve noticed it’s harder to read small print and focus your eyes on details, it could be time for an eye exam and reading glasses. But from an integrative perspective, there is so much more we can do to support eye health through menopause and beyond.

How Perimenopause and Menopause Affect Your Eyes

Age-related eye changes tend to start in the 40s for both men and women. You may experience:

  • Decreased focus in near vision (presbyopia)
  • Difficulty reading
  • Needing more light to see, especially for close tasks
  • Noticing more glare
  • Changes in color perception
  • Needing reading glasses or bifocals

Aging occurs in all eye structures, so some vision changes, and progression is common and even normal. However, menopause also plays a role.

There are estrogen receptors throughout the brain and in the eyes themselves. Estrogen plays a protective role, maintaining eye structure, function, and vision. Estrogen also protects against the development of eye diseases. There are also progesterone and androgen receptors in the eyes, as these hormones are important for eye health.

In perimenopause, estrogen often fluctuates with both high and low levels during the menstrual cycle. In later perimenopause and into menopause, estrogen levels fall and remain low. These low estrogen levels, or estrogen deficiency, can promote or accelerate changes in eye health and vision. This loss of estrogen (primarily estrogen but the other ovarian hormones as well) in menopause correlates with structural and functional changes in the eye and vision.

Estrogen deficiency accelerates optic nerve aging. It reduces retinal cells and nerve fibers, which can lead to more visual sensitivity. Estrogen deficiency also correlates with reduced tear production and higher intraocular pressure (fluid pressure within the eyes).

In one study, premenopausal women had lower intraocular pressure, along with lower blood pressure and heart rate compared to postmenopausal women. This observation makes sense when you consider the influence of low estrogen and menopause on cardiovascular health, and poor cardiovascular health can increase the risk for eye diseases.

Dry Eyes, Glaucoma and Other Eye Conditions During Menopause

Most of the research regarding eye health and menopause focuses on two conditions: dry eyes and glaucoma. The risk for both increases with estrogen deficiency and menopause.

Dry eye disease is a condition characterized by reduced tear production, eye strain, sensitivity, pain, visual disturbances, and inflammation. It’s most common in women over 50 (menopausal women) and twice as prevalent in women as in men.

Estrogen deficiency drives dry eye disease. Without as much estrogen binding in the eyes, the lipid layer in the eyes becomes thinner and eye tissues become less lubricated. The more severe dry eyes become, the more they impact a woman’s quality of life.

Dry eyes are also associated with blepharitis, a general condition of infection and inflammation in the eyelids and roots of the eyelashes.

Glaucoma is an eye disease where increased intraocular pressure leads to progressive damage to the optic nerve. It’s the leading cause of blindness globally.

Menopause increases glaucoma risk. Estrogen deficiency, early menopause, changes in estrogen signaling, and changes in estrogen receptors all correlate with glaucoma. Women who go through menopause at a later age seem to have more protection against glaucoma or a delayed onset.

Evidence-Based Ways to Support Eye Health During Menopause

When it comes to eye health through menopause and over the lifespan, there is so much we can do preventatively and proactively. Let’s explore the tools we have:

  • Schedule Regular Comprehensive Eye Exams. It’s good to have a baseline in your 40s, even if your vision is good. In perimenopause, repeat the eye exam every 1-2 years, and then annually after menopause or as recommended by your ophthalmologist.

 

  • Why Natural Outdoor Light Supports Eye Health. A walk in nature isn’t only good for fitness and nervous system health; outdoor light is also good for your eyes. Natural light helps support the circadian rhythm and sleep, and it can support healthy vision. Most of the research on outdoor light involves children and the prevention of myopia (nearsightedness). However, there are likely benefits for all.

 

  • Reduce Digital Eye Strain During Menopause. Working on the computer or reading on your phone can contribute to eye strain, dry eyes, blurry vision, headaches, and other symptoms.

 

  • Improve Lighting to Reduce Eye Fatigue. We often talk about dimming indoor lighting after dark to support sleep, but it’s also important, especially with age, to have enough light for your eyes to work optimally. You may need to brighten the space around where you work, read, craft, or use screens. This could be as simple as working by a window in the daytime.

 

  • Eat a Nutrient-Rich Diet That Supports Healthy Vision. Eating patterns help shape long-term health, including eye health. The good news is that many of the nutrition recommendations for your eyes are also helpful for other aspects of menopause and aging. Choose whole foods, lots of colorful produce and other plant foods, healthy fats, herbs, and spices. Also, stay hydrated.

 

  • Why Omega-3 Fatty Acids Matter for Eye Health. Omega-3 fats, particularly DHA, are critical for structure and function in the eyes, including lubrication. Eating foods high in omega-3s, like cold-water fish, is supportive of eye health. You can also take a daily omega-3 supplement.

 

  • Can Lutein and Zeaxanthin Help Protect Your Eyes? Lutein and zeaxanthin are carotenoid compounds with antioxidant actions and an affinity for the eyes. They are naturally found in vegetables and eggs and can also be taken as supplements. They may help delay the progression of age-related macular degeneration, cataracts, and potentially other eye diseases. Supplemental astaxanthin, another carotenoid, may also support and help manage eye disease.

Estrogen appears to have protective effects on the optic nerve, and declining estrogen levels have been associated with structural and functional changes within the eye.

Although menopause is one risk factor, glaucoma develops from multiple contributing factors, including age, genetics, cardiovascular health, and elevated intraocular pressure.
